Comments:
An unannounced full renewal inspection was conducted today. There were 25 children in care, with a total of 4 staff members assigned to their direct supervision. Children's records, children's medications, staff records, background checks, administrative records, classroom programming, staff:child interactions, emergency preparedness items, children's nutrition, and staff training were inspected today.
The children were observed creating winter art projects, assembling puzzles, practicing handwriting skills, enjoying circle time discussions, eating lunch, and engaging in creative free play activities. The children were served tomato soup, grilled cheese sandwiches, mixed fresh fruit, and milk for lunch today.

Based on a review of children's health records, it was determined that one record does not include documentation of a completed physical examination under the direction of a physician. Evidence: Child A has attended care for more than 30 days. This child's date of enrollment was 3/7/18. There is no documentation of a completed physical exam for this child.
Plan of Correction:
The parent was immediately contacted during the inspection. He will bring the physical report at pick up today.

Based on a review of administrative documents, it was determined that the center has not developed written procedures for injury prevention. Evidence: There is no injury prevention plan.
Plan of Correction:
I will create the plan and review the information with staff. We review injury reports at each staff meeting. In addition, we will also review the injury prevention plan.

Description:
Based on a review of administrative documents, it was determined that the center has not developed written playground safety procedures that include all requirements. Evidence: Staff responsibilities for maintaining an adequate level of resilient surfacing are not addressed in the playground safety plan.
Plan of Correction:
I will add the information to the playground safety plan, then review the updated information with all staff.
