Comments:
An unannounced monitoring inspection was conducted today from 11:00am-2:15pm. There were 119 children (infants-5yrs.) directly supervised by 43 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, listening to stories, participating in character education time, coloring, counting, practicing numbers, playing with small manipulative toys, learning about geometric shapes, playing outside, and eating lunch. Today's lunch consisted of Chicken Alfredo Pasta, mixed vegetables, fresh fruit and milk. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Based on record review, the center did not obtain a follow-up tuberculosis (TB) screening at least every two years from the date of the last TB screening or testing.
Evidence: Staff#1's record contains a TB screening that was last updated on 9/1/2017.

Description:
Based on record review, staff records were incomplete.
Evidence: Staff#1's record (DOH: 5/15/2018) is missing one of two references as to their character and reputation, as well as competency required prior to their first day of employment. Staff#2's record (DOH:10/23/2018) is missing two references as to their character and reputation, as well as competency required prior to their first day of employment.

We'll use the reference check service that we have to get their references back.

Description:
Based on review of medication, the center has stored long-term prescription medication with the child's name written on the medication, but the medication does not include a prescription label to identify the child, the prescriber's instructions pertaining to dosage, frequency, and manner of administration.
Evidence: A long-term medication observed on-site, for Child#1, did not have the prescription label attached to the medication.
Plan of Correction:
We'll contact mom to get a new label to place with the medication.
