22VAC40-185 STAFFING AND SUPERVISION.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. The monitoring inspection was initiated and concluded on 10/5/20. . The owner/director was contacted by telephone to conduct the investigation. The evidence gathered during the investigation supported non compliance with standards and law.
A violation was cited in the area of care, protection and guidance related to information submitted by the provider/licensee of a report of an injury to a child while under the supervision of a center/provider that requires outside medical treatment /death of a child in care. Please see Violation Notice for more details.

Based on a review of documentation and interview, it was determined that the center and its staff failed to ensure the care, protection and guidance of children in care. Evidence: On 10/2/20 at approximately 4:58 pm,13 school age children and one staff were outside on the playground. During this outside time, child A ( 7 years of age) got on the back of child B (12 years of age) for a "piggy-back" ride. At some point during the "piggy-back" ride, child A fell off child B's back and used their hand to brace the fall which resulted in a broke wrist.
Staff failed to intervene and redirect the children to appropriate play and failed to have the children adhere to the Virginia Department of Social Services (VDSS) COVID-19 guidelines and information document regarding children maintaining social distancing of six feet.
Plan of Correction:
Counseled children on appropriate playground play and reviewed the playground rules. Also counseled children on following COVID-19 guidelines and social distancing. Counseled staff on appropriate playground play and reviewed the playground rules. Also counseled staff on following COVID 19 guidelines and social distancing.
