Comments:
An unannounced monitoring inspection was conducted with the director on June 4, 2024, beginning at 1:07 p.m. and ending at 2:50 pm. There were 29 children, ages between 6 months and 5 years old, in the direct care of 5 staff. The children were observed during their resting/nap period. Five children's records were reviewed. Four staff records were reviewed and in compliance. No medications were on site to be reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent practice emergency evacuation drill was documented as 5/19/2024. The most recent practice shelter-in-place drill was documented as 1/24/2024. The most recent practice lockdown drill was documented as 6/13/2023. The most recent health inspection was dated 2/28/2023. The most recent fire inspection was dated 9/13/2023.

Based on review of documentation, the center did not ensure that each child had documentation of a physical examination within 30 days of the first day of attendance. Evidence: Child A did not have documentation of a physical examination in their record.
