10/24/19 Arrived 2:00pm Departed 5:15pm An unannounced monitoring inspection was conducted today. Fifty-three children were present with five staff. The children were doing homework, reading, playing games, playing in the gym and playing outdoors. Previous violations, medication, staff and children's files were reviewed. Based on the review of six staff files, the facility failed to ensure that references are checked for each employee. Evidence: 1. There are no references on file for staff #1,2,4, 6.

Based on the review of emergency evacuation documents, the facility failed to conduct monthly fire drills. Evidence: 1. There was no fire drill in September 2019.

The chief of staff will conduct monthly fire drills. Two drills will be conducted in October 2019.
