An unannounced renewal inspection was conducted today. There were 33 children present with six staff. The children were eating a snack, playing in the gym and having art class. Three child records and six staff records were reviewed. The playground is not in use until new resilient surface material is installed.

Based on the review of staff and board officer's records, the facility failed to ensure that all required background checks are completed within the required time frames. Evidence: 1. There is no out-of-state background check on file for board officer #3. (date position began 3-18-21)
Plan of Correction:
The Chief Operating Officer reported that the background check has been sent, twice, but has not been returned by the out-of-state central registry.
