Comments:
An unannounced monitoring inspection was conducted on September 11, 2023. This on-site visit was conducted from 11:00 a.m. through 12:00 p.m. Upon arrival, there were 45 children, ages 7 months through 5, in the care of 15 staff members. Five children and five staff records were reviewed.

Please complete your plan of correction within 5 business days and return to the licensing inspector.

Based on record review the facility failed to obtain fingerprints for one staff member prior the date of employment.
Evidence: 1. Record review revealed Staff # 4 did not obtain fingerprints prior to date of hire. 2. Date of hire was documented 11/22/2022. Fingerprint check was dated 11/28/2022.
