Comments:
A renewal voluntary registration inspection was conducted on March 20, 2024 from 1:20pm-2:30pm. There were two children present and one staff. The inspectors reviewed four children?s records and one staff record, along with any requested program records submitted by the facility to determine if required documentation was complete. Children were observed during free play. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit meeting.

Violations:
Standard #: 8VAC20-850-90-C
Description: Based on record review and interview the provider did not ensure to include all the required information in children?s records.
Evidence:
1.The record for Child #1 did not contain the child?s date of enrollment or a work phone number for one parent.
2.The record for Child #2 did not contain the child?s date of enrollment, both parents? work phone numbers, the name, address, and phone number of the child?s physician, proof of identity, or an immunization record.
3.The provider confirmed that the information for Child #1 and Child #2 listed above was not available for review.

Standard #: 8VAC20-850-110-C
Description: Based on observation the provider did not ensure to meet requirements of the health and safety checklist regarding safeguarding the home from hazards.
Evidence:
1.There were one-inch nails protruding from a wall in the playroom where children were actively playing.
2. The bathroom used by the children in care had sticky insect traps accessible to children.
