10/4/2019 Arrived 3:10 pm Departed 5:00pm An unannounced monitoring inspection was conducted today. Forty-seven children were in care with five staff and were eating a snack, doing homework and playing outdoors. Previous violations, seven children's files and four staff files were reviewed. Based on the review of four staff files, the facility failed to ensure that all background checks are conducted for each employee. Evidence: 1. There are no signed sworn statements on file for staff #1 and 2. Employee hire date 9/30/19.
Plan of Correction:
The chief of staff will ensure that staff #1 and 2 sign sworn statements immediately. Going forward all new employees will sign the sworn statements before employment.
