10/17/22 Arrived 2:45pm Departed 4:15pm An unannounced monitoring inspection was conducted today. There were 88 children present with six staff. The children were arriving, eating a snack and doing homework. Five child records and three staff records were reviewed.
The playground is not in use until renovations are complete.

Based on the review of three staff files, the facility failed to ensure that all employees have a completed central registry clearance on file within 30 days of hire. Evidence: 1. There is no central registry clearance on file for staff #1 and 2. (hire dates 8-12-22; 9-7-22)
Plan of Correction:
The Chief of Operations reported that the background checks were submitted to the central registry.
