4/13/23 Arrived 2:45pm Departed 5:05pm An unannounced renewal inspection was conducted today. There were 69 children (ages 5-12yrs) present with eight staff . The children were arriving, doing homework, playing in the gym and outdoors and eating a snack. Six child records were reviewed. Four staff records were reviewed at the main office.
You reported that the playground is not in use. Thank you for your cooperation. Please contact me if I may be of service.
Based on a review of staff files, the facility failed to ensure that all employees have a central registry search on file by the 30th day of employment. Evidence: 1. Staff #1 is working today and does not have a central registry check on file. Hire date 9-7-22. REPEAT VIOLATION
