Comments:
An unannounced monitoring inspection was conducted on 5/24/23 at 9:40am to 10:45am to investigate a self-reported incident regarding forbidden actions toward a child by a staff member. Interviews were conducted, the classroom was observed and documentation was reviewed. One violation was issued as a result of this inspection. An exit interview was conducted on 5/30/23. Standard #: 8VAC20-780-410-1
Description: Based on staff interviews, the center did not ensure that the forbidden action of physical punishment of a child did not occur.

Evidence:
Upon interview with staff at the center, it was reported that Child #1 was expressing challenging behaviors including throwing themself on the floor in a tantrum, throwing items in the classroom and screaming loudly. Staff #1 became frustrated with the child and pinched them on their arm as a form of punishment.

Plan of Correction: Center Manager will provide and review the Agency's Policies and Procedures for Positive Behavior Guidance again with all staff. The Agency will also continue to provide ongoing training opportunities for staff on strategies and techniques on how to support children with challenging behaviors and self-regulations during indoor and outdoor experiences and activities.
