Comments:
An unannounced renewal inspection was conducted October 30, 2023 between 9:30am and 11:30am. There were 4 children present with one (2) caregivers supervising within the supervision guidelines. The children were observed playing outside, diapering and toileting procedures, handwashing, and playing with other age-appropriate toys and materials. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, required posted documents, physical plant, staffing and supervision, programing, medication, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs. Based on observation and interview, the provider failed to ensure that wood burning fireplaces and associated chimneys are inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider. Evidence: 1. The provider did not have documentation of an annual inspection of the fireplace and chimney inspection. 2. The fireplace is open, though it shows no signs of being used.
Plan of Correction:
I had the top closed off years ago, but I don't know if I have paperwork. I will contact someone to get the fireplace inspected and permanently closed. I will never use it.

Description:
Based on observation the provider did not ensure pressure mounted gates are not used as protective barriers at stair opening. Evidence: A pressure mounted gate was observed at the bottom of the stairway opening leading to the second floor.

I will have someone to mount the gate to the wall.
