An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted today from 9:30 a.m. to 12:10 p.m. 23 children, ages 4 years through 5 years old, were under the care and supervision of 4 staff. Center documents and 4 staff records were reviewed. Based on review of documentation and interview with staff, it was determined that the center did not obtain sworn statements prior to the 1st day of employment and central registry background check results by the 30th day of employment. Evidence: Staff A, hire date 01/20106; staff B, hire date 10/2015; staff C, hire date 8/2010; staff D, start date 9/2018 were missing completed sworn statement forms and central registry background checks.
Plan of Correction:
In the future, all background checks will be completed within the designated time frames.
