Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 05/25/2021 and concluded on 06/02/2021. The director was contacted by telephone to initiate the inspection. There were 52 children present and 12 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on a review of four staff records, the vendor did not ensure two staff were evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.

Evidence: 1) The TB screening in the record for Staff #1, hired on 02/08/21, was dated 02/23/21. 2) The TB screening in the record for Staff #3, hired on 05/10/21, was dated 05/15/21. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Per the Vendor: "Corrected immediately. Moving forward Directors will use the check list attached to ensure all pre-hire requirements are complete. New hires will not be able to start until all items are on file."
