Comments:
An unannounced inspection was conducted today from 2:45pm to 4:00pm in response to a complaint that was received by the licensing office on 2/17/2020, regarding ratios and operational responsibilities. Based on interviews, records reviewed, and observations, it was determined that there wasn't a preponderance of evidence to support the allegation regarding operational responsibilities; however, the complaint was deemed valid with regards to ratios. A complaint related violation and non-related violations are identified on the Violation Notice. Based on record review, the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child#1's record contained an immunization record that was last updated on 4/25/2019.

Description:
Based on observation and record review, the provider did not follow the point system requirements to ensure that a caregiver did not exceed 16 points allowed per caregiver.
Evidence: Upon arrival to the Family Day Home at approximately 2:45pm, Caregiver#2 was present at the FDH with 11 children (31 points), exceeding the 16 points allowed per caregiver. Caregiver#1 arrived back at the Family Day Home 15 minutes after the inspection began.

At naptime provider leaves to pick up her kids. Will make sure points are within range.

Based on record review, it was determined that the provider did not obtain a fingerprint based background check for a household member residing in the home.
Evidence: HH#4's record did not contain the fingerprint based background check result required prior to the first day of the person residing in the home.
