Comments:
An announced inspection was conducted at the corporate office on Thursday, June 27, 2019 from 1:50 PM until 3:00 PM to review staff records. An administrator was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of staff records, the facility did not obtain a completed sworn statement for one staff member prior to beginning employment.

Description:
Based on a review of staff records and interview, the center did not obtain fingerprint results prior to hire for staff hired after 01/22/2018.
Evidence:
1. The record for staff #1 (date of hire 06/05/2019) had fingerprint results dated 06/18/2019, fourteen days after the hire date.
2. The administrator stated she was unaware that staff could not start employment even if they are just in training.
