Comments:
An unannounced monitoring inspection was conducted on 12/03/2019 from 10:00 A.M.-12:00 P.M. At the time of the inspection, six children were in the care of two staff members. Five children's records, three staff records, evacuation drills, first aid kit and supplies, health inspection, fire inspections, and required posted information were reviewed. Children were observed in a group literacy activity with letter sound and recognition, handwriting activity, handwashing, snacktime, free play activities that included dramatic play and building. The exceptions to compliance are noted on the violation notice.

Based on review of children files and attendance records, the center failed to ensure that immunizations were obtained before or on the first day of attendance.
Evidence: 1. Child #1 immunizations were dated 09/04/2019. First date of enrollment for Child #1 was 08/12/2019. 2. Child #4 immunizations were dated 08/23/2019. First date of enrollment for Child #4 was 08/13/2019.
Plan of Correction:
In the future the administration will discuss with parents that immunizations are required before a child can start attending the center.
