Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, January 14, 2020. The on-site inspection was from 10:55 a.m. to 1:35 p.m. The provider and an assistant were caring for three children (9 points). Lunch was observed to meet USDA guidelines. Five children's records and the record of the provider and a new assistant were reviewed. The provider, the assistant and two children were interviewed. There were no medications on-site or currently being administered, as is the policy of the provider. Some of the items reviewed and observed included: warm water temperature, first aid kit and emergency supplies, required posted information, updated vaccine for one dog, documentation of evacuation, lockdown, and shelter-in-place drills, refrigerator and freezer temperatures, choice time, infant care, diapering procedures, bathroom and handwashing procedures, TV time, lunch, and nap time.

Description: Based on record review and discussion with the provider, the provider failed to obtain a second reference as to character and reputation as well as competency of a new staff member.

Evidence:
1. The record for the new assistant hired did not contain documentation of a second reference. The new staff member began employment on 10/22/2019.
2. The provider confirmed that she needed to obtain a second reference for the new assistant.

Plan of Correction: The provider will obtain a second reference as soon as possible.
