Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was completed on December 4, 2020. The provider was contacted by telephone (video call) to initiate the inspection. There were four children and one resident child present and in the care of the provider and an assistant (11 Points). The inspector was able to view all required documentation. The provider's record, a staff member's record and two children's record were reviewed.
Information gathered during the inspection determined a non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the facility.
The Licensing Inspector reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of two children's records and verification from the provider, the provider failed to obtain all of the required enrollment information for two children.
Evidence: 1. The record for Child A did not contain the addresses of the two people the parent had listed as emergency contacts to be notified in case of an emergency and the parent could not be reached. Date of enrollment was 09/21/2020. 2. The record for Child B did not contain the address of one of the people the parent had listed as an emergency contact to be notified in case of an emergency and the parent could not be reached. Date of enrollment was 07/13/2020. 3. The provider confirmed that the required addresses were not in the children's files.
Plan of Correction:
During the inspection, the provider contacted the parents of the children and obtained the addresses for the emergency contacts. In the future, she will make sure to double check all enrollment information to ensure it is complete.
