A Subsidy Health and Safety Inspection was conducted on on 01/10/2023 from 10:20 a.m. to 12:20 p.m. There were six children present, ranging in ages from 15 months to 6 years old, with one staff supervising. Both inspections combined cover subsidy health and safety regulations.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all the subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the subsidy health and safety inspection and are identified on the violation notice.

Based on review of records and interview, the provide fail to maintain certification in cardiopulmonary resuscitation (CPR) that included an in-person competency demonstration. Evidence: 1. The provider?s CPR certification expired on 8/4/2022. 2. The provider completed an online CPR certification on 1/4/2023. 3. The provider confirmed that the was not an in-person competency test completed.
