Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 12/18/2020 and concluded on 12/18/2020 The director was contacted by telephone to initiate the inspection. There were 35 children present and 6 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Based on record review, 2 of 3 children's records did not contain all required parent contact information.
Evidence: 1. The record for Child #1 did not contain the name, address, and telephone number for the mother's school attended. 2. The record for Child #2 did not contain the address for both parents' place of employment.
Plan of Correction:
I will have the parents update the information during pick-up.
