08/25/2022 Arrived 10:05am Departed 11:45am An unannounced monitoring inspection was conducted today. There were seven children present with four staff. The children were playing outdoors and eating lunch. Two child records and four staff records were reviewed.
Information submitted by the program concerning care of children was reviewed. There was not a preponderance of evidence to support the allegations.

Based on a review of the playground, the facility failed to ensure that all equipment is maintained in a safe condition. Evidence: 1. On the small and large climbers, the rubber covering on the steps is worn and cracked. This presents sharp edges. 2. On the small climber, on the right side of the slide has a 1/4 -1/2 inch gap between the slide and the wooden board. This presents an entanglement hazard.
