Comments:
An unannounced renewal inspection was conducted on July 11, 2024. There were 71 children present, ranging from 5 months to 5 years, with 14 staff supervising. The inspection began at 10:40 AM and concluded at 1:05 PM. Children were observed participating in circle time, playing on the playground, playing with toys, and napping. All inside and outside areas of the childcare center were inspected. The center's first aid kit and non-medical emergency supplies were inspected and found complete. The required postings were inspected and found to be incompliance. Liability insurance, health inspection, and drills were all in compliance. Medications and authorizations were reviewed. There are staff on site with current First Aid, CPR, MAT, and DHO training. A total of seven children's records and seven staff records reviewed.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and interview, the center failed to ensure that all agents/board members shall undergo background checks every five years.
Evidence:
1. The record for Agent #1 contained a national criminal background check dated 4/26/2018. Agent #1 should have undergone a criminal background check on or before 4/26/2023.
2. The director verified that a criminal background check had not been completed for agent #1 since 2018.

Plan of Correction: Will update ASAP.

Standard #: 8VAC20-780-160-C
Description: (Repeat Violation) Based on record review and interview with staff, the center failed to ensure that all staff obtained a tuberculosis (TB) screening at least every two years from the date of the initial screening or testing.
Evidence:
1. The record for staff #2 contained an initial tuberculosis screening dated on 3/1/2021. Staff #2?s date of hire was 2/23/2021.
2. The record for staff #3 contained an initial tuberculosis screening date on 2/27/2022. Staff #3?s date of hire was 2/28/2022.
3. Staff #2 verified that an updated tuberculosis screening had not been completed for staff #2 and staff #3.

Plan of Correction: Will update within 30 days.

Standard #: 8VAC20-780-60-A
Description: (Repeat Violation) Based on review of records, the center failed to ensure that each child?s record contained all required information.
Evidence:
1. The record for child #4 (enrollment date 3/4/2024) and child #6 (enrollment date 3/4/2024), did not contain the parent?s work number.
2. The record for child #7 (enrollment date 4/8/2024) did not contain a work number for dad.

Plan of Correction: Notified of how to display when there is no work number for parent.

Standard #: 8VAC20-780-245-A
Description: Based on record review, the center did not ensure that staff completed the annual minimum of 16 hours of training appropriate to the age of children in care.
Evidence:
1.The record for staff #3 (hired 2/28/2022) did not contain documentation of the 16 hours of required annual training. There was only 2 hours of training documented for February 2022-February 2023.
2. The record for staff #4 (hired 10/14/2021) did not contain documentation of the 16 hours of required annual training. There was only 6 hours of training documented for October 2022-October 2023.

Plan of Correction: Staff notified and getting proof.

Standard #: 8VAC20-780-270-A
Description: (Repeat Violations) Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: A blue child size chair in the toddler classroom, was torn on seat of the chair exposing the webbing.

Plan of Correction: Thrown out and will be replaced.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medication and interview, the center did not follow the procedures for maintaining medication on-site.
Evidence:
1. In the Bumble Bee?s classroom, there was an EpiPen in a locked cabinet that had expired 6/2024.
2. Staff stated that they were not aware that the medication had expired.

Plan of Correction: Parent notified.
