Comments:
An unannounced renewal inspection was conducted on 05/09/2024 from 10:20am to 11:20am with the Provider. There were 4 children ages 15 months to age 3 years old (10 points) with the Provider. The programming area, attendance, outdoor playground area, menus, Provider record, 4 children?s records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating free play. The Provider had a current certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Based on observation and staff interview, the provider did not ensure that medication was kept in a locked place using a safe locking method that prevents access by children.
Evidence: An antibiotic ointment was stored on the table in an unlocked area as you enter the space where the children were playing.

I plan to ensure that all medication is locked in a safe place.

Description:
Based on observation, the Provider did not ensure that children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.
Evidence: There was no barrier to block children under the age of 2 years old from climbing or stairs to the deck outdoors next to the play structure.
Plan of Correction:
I plan to add a gate at the bottom of deck stairs.

Description:
Based on observation, the Provider did not ensure that the highest climbing rung or platform on outdoor climbing equipment or top of a slide does not exceed 6 feet for school age children and 4 feet for preschool children.
Evidence: The climbing structure that the children of all ages use at the home has a spiral slide and long straight slide that are 82 inches in height from the ground.
