Comments:
An unannounced monitoring inspection was conducted on 3/27/24 from 12:45pm to 2:15pm with program staff. There were 36 children in care, supervised by three staff. The children were observed eating lunch, doing a craft, and transitioning to the computer lab. Four child records and five staff records were reviewed. The Center had four staff on-site with current certification in CPR and First Aid, and three staff on-site trained in Daily Health Observation. One medication and authorization form were reviewed, and the center had two staff on-site current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 4/26/23 and the most recent Health Inspection on file was dated 3/18/24. Standard #: 8VAC20-780-80-A
Description: Based on observation, interview, and review of documentation, the center did not maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurred, for each group of children. Evidence: At approximately 12:52pm on 3/27/24, 13 eight- to nine-year-old children were observed in a classroom. The attendance for the group contained documentation of 14 children present.

Plan of Correction: Communicated with staff the importance of documenting arrival and departure times.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevented access by children. Evidence: On 3/27/24, three spray bottles containing cleaning materials were observed unlocked in the classroom where the five- to six-year-old children were observed. One spray bottle was observed on a high shelf, one was on a low toy bin, and one was on a tabletop.

Plan of Correction: Bottles containing hazardous substances have been removed and placed in the locked kitchen. Children do not have access to this kitchen.

Description: Based on observation, the center did not ensure that when hazardous substances were not kept in original containers, that the substitute containers clearly indicated their contents. Evidence: On 3/27/24, three unlabeled spray bottles containing a yellow liquid were observed in a classroom. The Assistant Director reported that the spray bottles contained cleaning materials.

Plan of Correction: Bottles containing hazardous substances have been labeled.
