An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted on October 17, 2019 from 11:32 AM and 1:15 PM with the director. Six children, ages 3 years through 4 years old, were under the care and supervision of 1 staff. Children were observed eating lunch prepared and brought from home. Center documents and three staff records were all reviewed.

Based on review of documentation, the center did not to ensure that satisfactory central registry background checks were obtained with 30 days of employment. Evidence:Staff A, start date 8/2004, staff B, start date 8/2017 and Staff C, start date 8/2017 did not have documentation of the sworn statement and a central registry background check results in their records.
Plan of Correction:
The center will ensure that all staff members have signed sworn statement and central registry background check completed immediately.

Description:
Based on review of documentation and interview with staff, the center did not provide written disclosure of required information to the guardians of the children in the center and general public. Evidence: There was no written statement regarding the physical facility, enrollment capacity, food services, health requirement and public liability insurance accessible to the parents and guardians in the center and the general public.
Plan of Correction:
The center will complete the Public Disclosure form and place it on a bulletin board so that it is accessible to the general public and parents and guardians of the children in the center.

Description:
Based on a review of documentation, the facility failed to obtain national fingerprinting background checks as required. Evidence: Staff A, B, and C did not have documentation of a completed fingerprinting background check in their records.
Plan of Correction:
The center will have all staff complete fingerprinting background checks for all staff and provide documentation to VDSS.
