Comments: An unannounced inspection was conducted beginning at 10:00 and concluding at 11:30 AM to investigate an injury to a child. Records were reviewed and interviews conducted regarding the incident. Violations: Standard #: 8VAC20-780-550-P Description: Based on information obtained from the injury / accident report, it was determined that the facility did not ensure that all required elements were present on an injury report. Evidence: The injury report did not contain spaces to document the time of the injury or any treatment provided. Plan of Correction: Facility was provided with an injury/accident report that contains all the elements required by the standards.
