54.1 Provider must be MAT certified to administer prescription medication.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review.

A renewal inspection was initiated on 9/24/2021 and concluded on 9/30/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 2 pet records, and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Adult 2's most recent Tuberculosis screening was conducted on 9/24/2019.

Description: Based on record review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

2. Adult 2 has received 14 hours of annual training since April 2020.
