Technical Assistance:
Forms for updating the Emergency Procedures Plan and a form for documenting Emergency Drills was emailed to the provider at her request.

Comments:
A monitoring inspection was initiated and concluded on 4/29/2022. There were two children present during the inspection, ranging in ages from 1 1/2 years to 7 years old, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 1 caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-110-C
Description: Based on observation and interview, the provider did not ensure that all criteria on the health and safety checklist are met when children are in care: "I keep medications and toxic household products in areas inaccessible to children and away from food products". The inspector observed a variety of toxic household products which were accessible to children during the inspection.

Evidence:
1. There were 2 children, ages 1 1/2 years old and 7 years old, in care during the inspection.
The licensing inspector observed a pump bottle of hand sanitizer, an aerosol bottle of air freshener on top of a low set of file drawers located in the kitchen, which is open to the living area where children were being cared for. The labels on these products stated "keep out of reach of children" and also contained additional warnings. These items were accessible to the children in care.
There were additional toxic household products containing warning labels located under the sink in the restroom that children utilize when they are in care. The items observed included Toilet bowl cleaner, a gallon sized bottle of bleach, a 2 liter bottle of hand sanitizer, a spray bottle of disinfectant. All of these products contained labels that read "keep out of reach pf children" along with additional warnings.
The floor level cabinet was not secured with a lock or child proofing mechanism.

2. "I make sure that any adult... who comes in contact with children or will provide care to children has a Tuberculosis (TB) test or screening..."
The TB screening for caregiver #1 was expired. The documentation was dated 3/12/2019 and TB screenings require updating every 2 years.

Plan of Correction: 1. Items were temporarily set out of reach of children during the inspection.
The provider stated that she would rearrange household items to ensure that those containing a warning label are stored, long term, in an area where children are not able to access them.
2. The provider was already making arrangements to have a new Tuberculosis screening conducted and will send the results of the screening to the licensing inspector upon receipt.
