Technical Assistance:
The VR Provider must send evidence to the licensing inspector that the background checks have been requested no later than 10 days of this notification dated 3/29/2024. The applicant must also then forward the results to the licensing inspector upon receipt.

Today we discussed the storage of medication and chemicals and the need for these items to be inaccessible when children are in care. Storing these items in rooms that the children do not use does not meet the requirement UNLESS these rooms are locked or unable to be accessed by children when they are in care.

Comments:
An unannounced monitoring inspection was conducted on 3/22/2024 from 2:10 pm - 3:45 pm. At the time of entrance, there were no children in care with the provider. Children were expected to arrive after school, however no children arrived during the inspection on this date. The sample size consisted of 2 newly enrolled children?s records and 3 previously viewed children's records. One provider Record was reviewed. The home was inspected as well as the front yard where children normally spend outdoor time. The provider stated that no medication is being administered at this time and children are not transported in a vehicle during hours that care is provided in this home. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 22.1-289.036-A
Description: Based on a review of records and interview, the provider did not obtain an updated Criminal History Report every 5 years thereafter.
Evidence:
One provider record was reviewed.
The Criminal History Results for Provider #1 was dated 7/11/2028.
The Criminal History check had not been obtained again within 5 years.
The provider stated that she is aware that the report needs to be updated and has been unable to travel to have new fingerprints taken due to personal reasons.

Plan of Correction: The provider will schedule fingerprints to be taken at the earliest date she is able to due to personal situation - with the goal of having them completed within 10 days or as soon after as possible.

Standard #: 8VAC20-850-110-C
Description: Based on a review of records, the provider did not always meet the criteria on the Voluntary Registered Health and Safety Checklist.
Evidence:
Five children's records were reviewed.
The provider was unable to demonstrate that she kept a copy of the physical examination reports for child #1.
The provider was unable to demonstrate that she kept proof of adequate immunization for child #1.
The provider acknowledged that she did not have this documentation for child #1.

Plan of Correction: The provider will speak with child #1's parent today and require that child #1's immunization record and physical forms be completed and returned to the provider within 10 days.
