Standard 480.B reviewed:
The center shall maintained a statement from the parent advising of a child's swimming skills before the child is allowed in water above the child's shoulder height.

The complete communicable disease statement required by Standard 90.A.3 needs to be added to the written parent agreement. The one on the new agreement lacks the statement in its entirety.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 07/16/2020 and concluded on 07/21/2020. The director was contacted by telephone to initiate the inspection. There were 125 children present and 12 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards and the violation was documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the director COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. Documentation of required immunizations were not available for child 1 who was in care during the inspection.
a. Child 1 has an enrollment date of 07/13/2020.
2. The program director confirmed that immunization documentation was not on file for child 1.

Plan of Correction: The immunization record for child 1 has been requested from the parent on the date of the inspection.
