A complaint investigation was conducted beginning on August 26, 2019 at 12:15 p.m. regarding physical plant, rest mats/cots, and programs: parental involvment, food policies, termination of services. Staff interviews were conducted. Documentation was reviewed. The playground was observed. The complaint regarding physical plant, rest mats/cots, programs: parental involvment, food policies and termination of services could not be substantiated.
Based on observation, not all areas of the center were maintained in safe, operable condition. Evidence: On August 26, 2019, the preschool/school-age playground back section of fencing was down . The infant/toddler playground's front section of fencing was down. The Licensing Inspector observed this initially on June 11, 2019 and it has not been remedied.
Plan of Correction:
We have placed caution tape and danger notice around the perimeter and placed a padlock on the entrance. Insurance claim was denied so we have to use school funds to repair. We will replace the damaged fencing by 10/25/2019.
