An unannounced complaint investigation was conducted on in response to a complaint regarding supervision and care, protection, and guidance. The investigation was conducted on March 20, 2024 with the director, beginning at 11:34 a.m. and ending at 2:31p.m. Interviews were conducted with the staff relating to the allegations. Documentation was reviewed. The allegations were regarding care, protection, and guidance were determined to be valid.

Based on a staff interviews, the facility failed to ensure the care, protection, and guidance of children that the staff were supervising. Evidence: It was determined through staff interviews, facility staff member were aware of continuous issues between two children. On 3/14/2023, there was conflict between two school age children that escalated to the point of one child hiding another child?s shoe around the classroom and a child smashing grapes on another?s foreheads while being supervised by facility staff.
