Comments:
An unannounced, on-site renewal inspection was initiated on June 11, 2024 and the inspection concluded on June 12, 2024. The on-site inspection began at 1:30 p.m. and ended 5:00 p.m. The inspector reviewed compliance in the areas listed above. There were 195 children present and 26 staff supervising. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation of a fingerprint based national criminal check shall be obtained for each staff prior to employment. Staff 5 (date of employment 5/28/2024) did not have a fingerprint check until 6/03/2024, Staff 6 (date of employment 5/28/2024) did not have a fingerprint check until 6/05/2024, and Staff 7 (date of employment 5/28/2024) did not have a fingerprint result on file.
Plan of Correction:
Staff will not be hired until the national fingerprint check is returned with satisfactory results.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child 9 (date of enrollment 6/03/2024) did not have documentation of an immunization record on file.

Description:
Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 6 (date of enrollment 8/7/2023) and Child 8 (date of enrollment 7/24/2019) did not have documentation of a physical.

Description:
The center shall ensure that procedures for administering medication shall include methods to prevent use of outdated medication. The emergency medication for one child expired 5/31/2024 and the parent had not been notified that a replacement was needed.

The parents will be notified that the medication shall be picked up within 14 days or the medication will be discarded.

Description:
Medication shall be labeled with the child's name, the dosage amount, and the time or times to be given. One medication designated for Child 1 had another family member's name and dosage amount and times specific to the family member.

The medication was returned to the parent.
