3/1/23 Arrived 9:30am Departed 11:35am An unannounced monitoring inspection was conducted today. There were 54 preschool children (ages 14m-5yrs) present with 11 staff. The children were playing outdoors, playing in centers and having group time. Five child records were reviewed. Staff records were reviewed at the main office. Thank you for your cooperation. Based on a review of staff files, the facility failed to ensure that employees who indicate having lived outside Virginia in the last five years, complete out-of-state background checks. Evidence: 1. There is no out-of-state central registry check on file for staff #3 (hire date 8-8-22) or for staff #8 (hire date 5-24-21)

Description:
Based on a tour of the playground, the facility failed to ensure that all equipment is maintained in safe condition. Evidence: 1. On the climber where the green slide attaches to the deck, there is a gap of 1/4 inch. This presents an entanglement hazard. REPEAT VIOLATION
Plan of Correction:
The site supervisor reported that the bolts on the slide had been tightened after the last inspection. They will be tightened again.
