Comments:
An unannounced monitoring inspection and facility tour was conducted from 2:05 pm-3:20 pm. At the time of inspectors' arrival there were 38 infant to preschool aged children in care with 5 staff members and 2 administrators. Children were observed participating in nap, indoor play, handwashing and toileting, and transitioning between activities. A sample of 5 children's records and 6 staff records were reviewed. Medications and injury reports were also reviewed. The areas of noncompliance were discussed with the directors during this inspection and are detailed on the violation notice.
Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the Sesame Street, Little Rascals, and Pooh & Friends classrooms/restrooms the cove base is separating from the walls. 2. In the restroom in the Pooh & Friends class there were two 1/2" holes in the wall behind the toilet. 3. In the restroom in the Little Rascals class there was exposed rust on the bathroom partition. 4. On the outdoor playground there was a tricycle missing end caps on the handlebars. 5. Ceiling tiles in several areas of the building were stained and sagging.
All of these areas were being utilized by children in care during this inspection and presenting potential injury hazards to the children in care.

Description:
Based on observation it was determined the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: The electrical room was unlocked, inside there was bleach, insecticide, and cleaning agents.
Plan of Correction:
Staff will ensure the electrical room stays locked at all times.
