Comments:
A monitoring inspection was conducted on April 10, 2024 to review voluntary registration requirements. There was one child present, age 2 years, with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of 3 children?s records and 1 caregiver record were reviewed. Violations found are noted on the violation notice.
Based on review, provider did not ensure 3 of 3 child records have documentation of required information.
Evidence: The following was found during review of child records: 1. The record for child #1 (start day 8/24/2023) did not have an address for the emergency contacts. 2. The record for child #2 (start day 1/17/2024) and child #3 (start day 1/17/2024) did not have an physical examination and immunization record.
Plan of Correction:
Provider will get information from parent today.

Description:
Based on observation and interview, provider did not ensure the criteria on the health and safety checklist are met at the time of inspection.
Evidence: The following was found during inspection: 1. There was not a protective barrier at the bottom of the stairwell leading to the second floor. The stairwell was accessible to child #1 who was present during inspection. 2. Upon arrival to the home at approximately 9:45 am, provider asked the inspector from window to give her a few minutes before answering the door. Once inspector was inside the home, provider stated the reason for the delay was because she was asleep. Child #1 (age 2 years) was present. Provider stated the child was asleep as well. Provider did not ensure children are properly supervised at all times.
Plan of Correction:
1. Provider will keep safety gate installed at stairwell entrance. 2. Provider will remain awake when children are present except during evening and overnight care.
