Comments:
An unannounced monitoring inspection was conducted today from 12:00 pm-1:00 pm. There were 9 children directly supervised by 3 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed eating lunch reading, and preparing for nap. Diapering procedures were also observed. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based on record review, documentation of orientation was not completed as required.
Evidence: Staff #1 and Staff #2 did not have documentation of staff orientation.

We did orientation. I will print out form and have them sign now and send to you.

Description:
Based on observation he diaper changing surface was not non-absorbent. Evidence: The diaper changing pad was not able to be cleaned and sanitized and considered non-absorbent due to a tear in plastic.
Plan of Correction:
I will buy a new changing pad.
