Comments:
An unannounced renewal inspection was conducted today from 10:25am-12:30pm. There were 12 children (35 points) supervised by 3 direct care staff. The home, 3 staff records, 8 children?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed coloring, playing with playdough, playing in home living, eating lunch, and engaging freely. Lunch served was chicken nuggets, cucumbers, apple slices, and milk. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Based on observation, 2 of 4 children's bottles were not labeled with the child's full name and date.
Evidence: Child #4's (5 months old), and Child#12's (5 months old) bottles were not labeled with the children's names and date.

Evidence: 1. Child #9's (3 years old) and Child #12's (5 months old) records did not include documentation of a physical form completed by a physician. 2. Child #11's (2 years old) record did not include documentation of the medical insurance policy number.
Plan of Correction:
I will get the policy number today, and will have the parents bring the missing physicals by next week.

Description:
Based on record review, 1 of 3 caregiver records did not include documentation of a current report of TB screening every two years from the date of the first screening.
Evidence: Caregiver C's most recent TB screening was dated 8/31/2017 (expired: 8/31/2019).

We will have the TB test completed by next week.
