Comments:
An unannounced on-site monitoring inspection was conducted on 08/09/2023. There were 5 children, ages 2 years-4 years, at a total of 8 points with the licensee during the inspection. The inspector observed the following: free choice activities, lunch and nap. A total of 4 children?s records and 1 staff record, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 10:15 AM and departed at 12:30 PM.

Based on observation the licensee failed to ensure that the tables were cleaned after each use or at least daily.
Evidence: 1.The children were observed doing an activity at the tables prior to lunch. The lunch was served, and the tables were not cleaned prior to eating.

The licensee will make sure that tables are cleaned after each activity.

Description:
Based on observation of nap time, the licensee failed to ensure that each child napping had a mat, cot or bed.
Evidence:
Three children napping in the back room was sleeping on a play mat that used as the flooring for the room. The children did not have an individual mat for nap time.
Plan of Correction:
The licensee purchased new mat, they are in her vehicle.
