Comments:
A renewal inspection was conducted on 4/05/2023 with the provider. There were 5 children present, ranging in ages from1 year to 2 years, totaling 15 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 caregiver/household member records were reviewed. The children were sitting in individual spots on the carpet watching TV and playing blocks. All children were picked up during the inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five children's records, the provider failed to have documentation of providing each parent in writing with all required information. Evidence: five children's files did not have the written policy on medication or the second page of the VDOE model form titled "Information for Parents" or an equivalent.

Description:
Based on review of five children's files, the facility failed to obtain documentation that each child has been adequately immunized prior to attending the child care. Evidence: Child 1 (date of enrollment 1/2/2023) and Child 5 (date of enrollment 1/2/2023) did not have documentation of an immunization record on file.
