Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the setting.
A monitoring inspection was initiated on October 27, 2021 and conducted on November 4, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from 7 months to 2 years, with 2 staff supervising for a total of 22 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation and interview. it was determined that one infant's bottles were not fully labeled with the child's full name and formula was not labeled. Evidence: 1, Infant B has bottles that had masking tape on that only showed part of the name. The formula container also did not include a label to indicate who it was for.

I have the others labeled and dated and will work on this.

Description:
Based on record review of 2 children's records it was determined that one did not contain a parent's work number. Evidence: 1. Child 1's enrollment information contained work address for one parent but it didn't contain a work number.
