Comments:
The licensing inspector conducted an unannounced monitoring inspection that began at the center at 9:00 am and concluded with staff record reviews at the central office at 11:45 am. There were fourteen preschool children in care with two staff. The children were observed playing in the centers, listening and moving to music. Two staff's records and five children's records were reviewed. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records.
Evidence:
1. The inspector reviewed five children's records. Two of five records reviewed (children's records #1 and #2) were missing the following information: child record #1 was missing the street address for one emergency contact and child record #2 was missing proof of identity and age (child #2's date of enrollment was 08/20/2019).

Plan of Correction: Child record #1 will be fixed immediately. FRS called parent of child #2 to remind to send birth certificate.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center failed to have 6 inches of resilient surfacing on the playground.
Evidence:
1. The inspector observed the playground located behind the school. The resilient surfacing (pea gravel) measured 1 - 2 inches in the fall zone surrounding the mats at the base of the fort. The fall height of the fort is approximately 40 inches.

Plan of Correction: Pea gravel will be added to meet regulations of 6 inches.
