Comments:
The licensing inspector conducted an unannounced renewal inspection from 9:00 am - 11:30 am. There were fifteen preschool children in care with two staff and one volunteer. The children were observed playing in the centers, at story time, singing songs and preparing for lunch. Two staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. If you have any questions, please contact your licensing inspector at (276) 608-3758. Thank you for your cooperation.

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center failed to have six inches of resilient surfacing on areas of the playground.
Evidence:
1. The inspector observed the playground located behind the school and measured the resilient surfacing (pea gravel). The resilient surfacing measured approximately three to four inches in the fall zones surrounding the mats at the base of the fort. The fall height of the fort is approximately forty inches.

Plan of Correction: The teacher contacted the central office during the inspection to inform them that additional pea gravels are needed for the playground. The central office said pea gravels were added to these areas since the previous inspection; however, additional pea gravels will be added to increase the height of the resilient surfacing to six inches.
