An unannounced inspection was conducted on 01/08/2024 from approximately 9:17 a.m. ? 10:16 a.m. The inspector reviewed the areas of non-compliance identified in the Intensive Plan of Correction (IPOC). The information gathered during this inspection determined continued non-compliance of the applicable standards. Repeat violation Based on record review, the provider failed to ensure that all applicants for employment shall submit to fingerprinting and undergo a national criminal background check prior to employment. Evidence: The record for staff S2, employed 03/13/2023 did not have results of a national criminal background check prior to the first day of employment. The file did not contain results of a national criminal background check.

Based on staff record review, the provider failed to obtain the results of a sex offender registry search and out of state child abuse and neglect search from any state in which the individual has lived in the preceding five years.
Evidence: Staff records for S8 and S9 documented on the sworn statement having lived out-of-state in the last five years. The records for S8, employed 08/21/2023 and S9, employed 11/21/2023 did not contain a sex offender searches for two states in which the individuals lived in the preceding five years and the results of the out of state child abuse and neglect searches.
