Comments:
An unannounced renewal inspection was conducted on 5/2/2023 from 9:45 am-10:55 am. There were 9 children (22 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed eating and napping. Areas of non-compliance are identified in the Violation Notice. Based on observation and interview, it was determined that the provider is not maintaining a written record of children in attendance each day.
Evidence: There was no attendance maintained for 5/2/23. There were 9 children present.

Description:
Based on record review, the provider did not orient their assistants by the end of their first week of assuming job responsibilities.
Evidence: 1. There was no documentation of caregiver #2 (DOH: 8/15/2016) and caregiver #3's (DOH: 4/2/2014) orientation training.
2. The provider stated that they did orient both caregivers, but threw away their orientation documents because they believed the caregivers required an updated orientation form each year.
