An unannounced monitoring inspection was conducted on 11/9/2023 from 10:00 am-10:45 am.
There were 7 children (20 points) supervised by 3 direct caregivers. The home, 3 caregiver's records, 4 children?s records, 1 household member record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing outside and napping. Areas of non-compliance are identified in the Violation Notice.

Based on record review and interview, the provider did not annually review with the parent the emergency contact information to ensure the information was correct and obtain the parent?s signed acknowledgment of the review.
