Description:
Based on observation, areas and equipment of the center, outside, were not maintained in a safe and operable condition.
Evidence: 1. There were two metal chairs on the playground that were rusting and had chipped and peeling paint. 2. There was cracked plastic on the windowsill of the log cabin play structure on the playground. 3. There was cracked plastic on both benches of the plastic picnic table located on the playground.
