Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 09/28/21 and concluded on 09/28/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 10 children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-90-A
Description: Based on a review of two children's records and interview, the provider did not obtain documentation that one child had been adequately immunized before the child attended the family day home.

Evidence: 1) The immunization documentation in the record for Child #1, enrolled on 08/16/21, was dated 08/24/2021. 2) During interview, the provider reported the immunizations documentation was not provided before Child #1 attended the family day home.

Plan of Correction: The immunizations have been obtained. The family recently relocated to the area and parent provided the immunizations as soon as they could. In the future, immunizations will be required prior to attendance.
